Your Rights and Repsonsibilities

As members of the practice team we are committed to giving you the best possible service. As a patient you are entitled to being treated with courtesy and respect by named members of staff. To be given the opportunity to choose whether to be involved in research or medical training and to see your health records subject to limitations in the law. You are entitled to have appropriate drugs and medicines prescribed and have a clear explanation of any treatment proposed and to being referred to a consultant acceptable to you when your doctor thinks it necessary and to be referred for a second opinion if you and your doctor agree this is desirable. You are entitled to be treated as an equal and be offered a health check on joining the practice

If you would like to provide feedback, complain or make a suggestion, you can ask to speak to or make an appointment with our Operations Manager. If you prefer to let us know in writing you can download an Incident Report form by clicking here, collect from reception or any of our waiting rooms. Please then submit to the Operations Manager who will provide a response within 14 days of receipt.

The practice team has made certain undertakings to you. In return you are asked to extend the same courtesy and politeness to the practice team as you would expect to receive. Attend appointments on time and tell us as soon as possible if you cannot attend. We ask that you hand in your repeat prescription requests or post them in good time. Inform us if you change your address or telephone number. Please only make Out of Hours calls if absolutely necessary.

Please note that the doctors reserve the right to remove a patient from their list if the patient feels unable to meet the responsibilities outlined in the section above or if the behaviour of a patient is violent or abusive to staff either on the premises or at the place where treatment is provided.
